Preparing for a D.O.T. exam involves gathering several important documents and medical records. Here’s a checklist to help ensure you have everything you need:

What to bring to your D.O.T exam:

• Driver’s License: Bring your valid driver’s license.

• Form of Payment: Ensure you have the necessary form of payment for the exam fee.

• List of Medications and Diagnoses: Prepare a detailed list of all current medications and medical diagnoses.

• Diabetic Patients: Bring your most recent hemoglobin A1c printout.

• Sleep Apnea Patients: Provide your CPAP compliance printout.

• High Blood Pressure: Bring documentation showing your blood pressure readings taken within the last 30 days from your treating physician.

• Past Heart Attack: Obtain a cardiologist release. Contact us for a clearance form prior to your appointment.

• Past Coronary Bypass: Obtain a cardiologist release. Contact us for a clearance form prior to your appointment.

• Past Angioplasty/Stent: Obtain a cardiologist release. Contact us for a clearance form prior to your appointment.

• Pacemaker: Contact us for a clearance form prior to your appointment.
