Do You Need a DOT Physical?
You need a DOT medical examination and must carry a valid medical certificate if any of these apply to you:
- You hold a CDL or operate a commercial motor vehicle over 10,001 lbs GVWR in interstate commerce
- You drive a passenger vehicle designed to carry 16 or more passengers (including the driver)
- You transport 9–15 passengers for hire beyond 75 air miles from your regular work-reporting location
- You haul hazardous materials in quantities that require placards

What Happens During Your DOT Physical
Your exam covers four things:
1. Health History Review
We’ll go through your medical history, current medications, and any conditions that could affect your ability to drive safely. Be honest and thorough — this protects you and speeds up the process.
2. Vision & Hearing Tests
DOT requires 20/40 correctable vision in each eye. Glasses and contacts are fine — just bring them. We’ll also check that you can hear a forced whisper at 5 feet (or pass an audiometric test).
3. Physical Examination
Blood pressure, heart, lungs, abdomen, spine, reflexes, and neurological function. Your blood pressure must be under 140/90. If yours is medically managed, you can still pass — but your certification may be limited to one year.
4. Urinalysis
A urine sample screens for underlying conditions like diabetes and kidney issues. This is not a drug test — that’s a separate process handled through your employer.
Once you pass, you’ll walk out with your Medical Examiner’s Certificate that day. No waiting for results.
Bring Your Documentation — We Can’t Start Without It
If you have a monitored medical condition (diabetes, sleep apnea, heart condition, recent surgery, hernia, workers’ comp claim, or recent back injury), you MUST bring clearance documentation from your treating physician.
We cannot place exams on hold or amend results after the fact. All clearance letters must be in hand at the time of your exam. Missing documentation means you’ll need to reschedule — and nobody wants to make that drive twice.
Also bring: valid driver’s license, glasses or contacts if you wear them, hearing aid if you use one, and a list of all current medications with dosages.
Quick Note on Diabetes & Blood Pressure
Diabetes controlled by diet or oral medication is permitted, but may limit your certification to one year. Some insulin-treated drivers need an endocrinologist waiver. Blood pressure over 140/90 at time of exam requires medical management documentation. Use of any Schedule I controlled substance disqualifies you. If any of this applies, call us before your appointment and we’ll tell you exactly what to bring.
Heart Conditions Require Extra Documentation
If you have a current diagnosis of cardiac insufficiency, congestive heart failure, or any other cardiovascular condition, you’ll need to provide a stress test performed within the last 12 months along with a physician’s release stating you can operate a commercial vehicle without restriction. Most drivers receive a 2-year medical examiner’s certificate. However, if you have a condition that requires monitoring — like managed blood pressure, diabetes, or sleep apnea — your examiner may issue a shorter certification (1 year, 6 months, or 3 months) so we can keep tabs on your health.
Once you pass, you’ll receive your Medical Examiner’s Certificate on the spot. You must carry a current copy with you whenever you drive. We also report your results to the FMCSA national registry as required.
